Steps to Fill Out an Application
Filling out the Application for Magnet Schools or Open Enrollment.
Follow the numbered boxes on the application.
– List Student Information
Please print clearly and neatly.

If the student is currently enrolled in FUSD, they have a Student ID number. Please print this number in the top box. If the student is entering Kindergarten or applying from outside the district, they will not have an ID number assigned and this box should remain blank.

Complete all the remaining information, including the grade level requested for next year.
– List the next school year
Most applications are for the beginning of the next school year. (Example: 2008 – 2009)
– Choose 1 or 2 schools
Magnet Schools allow you to choose up to two schools in one category - High Schools, Middle Schools or Elementary Schools. Be sure to label the schools 1 for first choice and 2 for second choice. If applying to the criteria schools, indicate that school as your first choice and go back to the appropriate Middle School or High School section to indicate a second choice, if applicable. Enrollment changes in high school programs can only be made at the semester to maintain appropriate class credits.
Open Enrollment allows only one choice from the High School, Middle School or Elementary School Section. Place a check mark in front of that choice.
– Parent Signature and contact information
The application must be signed by the parent or guardian to be valid. Contact information is essential.
– Submit your application
Mail, FAX or deliver the application to FUSD Student Services. Schools will also accept applications and forward them to the appropriate office. Call 457-6063 for more information.
